

The Community Outreach Clinic at Community Memorial Hospital serves those who are uninsured and underinsured in our community and surrounding areas. In 2011, the Clinic served 2,040 patients.
The Clinic provides immediate care and treatment for patients’ health conditions or problems, assesses their needs, and provides referrals to appropriate professional or community resources. The Clinic also provides case management to help chronically ill patients live healthier lifestyles and successfully manage their illness, thus reducing unnecessary emergency room visits and the need for more serious and costly medical treatment. Treating primary care patients at the Outreach Clinic saves the community an estimated $100,000 in emergency room costs each year.
Appointments at the Outreach clinic are available Tuesday and Thursday evenings from 5-8 pm. To make an appointment, call 262-257-3393. Walk-in patients will be accepted between 5:30 — 6:30 pm only.
