Hospital Maps
If you're coming to Community Memorial Hospital, the maps below will help
you find your way from Town Hall Road to the reception area in the following
departments.

Area Map
From the north:
- Take Hwy. 41/45 south to the Main Street Exit, drive west to Menomonee Falls.
- Follow Main Street approximately two miles west to Town Hall Rd.
- Turn left (south) on Town Hall Rd. and follow it approximately three quarters of a mile to Community Memorial, on the right (west) side of the road.
- The hospital sits on a hill. LindenGrove Health Care Center is located in front of the hospital.
From the south:
- Take Interstate 894 to Hwy. 45 north (to Fond du Lac). Continue on Hwy. 45 north for approximately 10 miles.
- Take the Appleton Ave. North Exit (Menomonee Falls). The exit curves underneath Hwy. 45. Stay on Appleton Ave. until you reach the first set of stoplights, which is the intersection of Good Hope Rd. and Appleton Ave.
- Take a left turn and travel west three miles until you reach Town Hall Rd. Turn right and continue for one mile.
- The hospital sits on a hill on the left. LindenGrove Health Care Center is located in front of the hospital
In addition to the main hospital, Community Memorial Hospital offers a Rehab & Sports Medicine Center.
