Confidentiality Agreement for Students. Instructors must ensure that each student submits a signed agreement at the beginning of a CMH Clinical.
Student/Instructor Parking Roster. Each student must sign this form. The completed form must be submitted at the beginning of the clinical experience.

Requesting a Nursing Student Experience at CMH. This document contains guidelines and due dates for requesting a clinical, precept/mentor or observation experience.
