Community Outreach Health Clinic is located in the Outpatient Care Center of Community Memorial Hospital.

The Community Outreach Health Clinic is located in the Outpatient Center at Community Memorial Hospital.

Hours

Monday 10:00 am – 2:00 pm
Tuesday 5:30 pm – 8:00 pm
Thursday 5:30 pm – 8:00 pm

Appointments at the Outreach clinic are available Tuesday and Thursday evenings from 5-8 pm. To make an appointment, call 262-257-3393. Walk-in patients will be accepted between 5:30 — 6:30 pm only.
