Services Provided
Services provided on-site at Community Outreach Health Clinic are free. Further referral services such as lab, x-rays, medications, treatments and therapies may be subject to charges through those providing the services.
- Care is provided by appointment or on a walk-in basis.
- Primary services focus on treatment, screening, education and referral for medical and dental care.
- Wisconsin Well Women Program for preventive health screening to eligible women between the ages 45 – 64.
- The Clinic assists people in obtaining a one-time referral visit to health and dental care and to appropriate agencies and services.
- A limited supply of medications is provided.
- Provide enrollment for Prescription Assistance Programs.

The Community Outreach Health Clinic assists persons in obtaining ongoing health and dental care by referral to appropriate agencies and services including:
- Specialty medical care
- Diagnostic testing
- Psychological counseling
- Definitive dental care
- Pregnancy testing
- Social services
- Support groups
- Other county or local community agencies
