2010 Summer Volunteen Program
Please read the following information carefully!
The Community Memorial Hospital Volunteen Program starts on Monday, June 14, 2010 and ends on Saturday, August 14, 2010.
Volunteens will be committed to volunteer on the average of once per week for a 4-hour shift.
The Volunteen Program provides general volunteer experience in a hospital setting. It is not set up to provide general or specific healthcare experience.
Applications will no longer be mailed. Returning Volunteens and Volunteens who are accepted from the waiting list will receive information on the dates/times to come in to fill out their applications and related forms.
College students will be accepted on the waiting list; however, returning and new high school Volunteens will be given priority.
Eligibility:
- 15 years of age or older.
- Have completed freshman year in high school.
- Commit to 7 weeks of the 9 week program.
- Can start the week of Monday June 14, 2010.
- Can complete required 7 weeks by Saturday, August 14, 2010.
- Can attend 1 of 3 sign-up sessions.
- Can attend the Volunteen Orientation on Monday, May 17, 2010 from 3:30 pm to 8:00 pm.
- Bring the following when they come to fill out their application:
- Complete immunization records of the following:
- at least two MMRs (measles (rubeola), mumps, and German Measles (rubella))
- proof of immunity to Chicken Pox (immunization or history).
- Information on known dates for vacations/ trips out of the country / school activities / study abroad. Volunteens who cannot volunteer for seven of the weeks between June 14 and August 14 because of other commitments will not be eligible for the program.
- Be prepared to provide health history information at the sign-up session (e.g. allergies, health issues, limitations / restrictions).
You must complete the Volunteen Health Record Form in its entirety, as part of the sign up session application process. The heath record form can be filled out when you come in to apply for the Volunteen Program or you can download the PDF copy, fill it out completely, and bring it with you. Applications will not be accepted without an entirely completed Volunteen Health Record Form that includes the required immunization history for childhood illnesses.

on forms that are part of the application process.

Applications will not be accepted without the presentation of the complete immunization records, a completed health history form on the day of sign-up, or without the accompaniment of a parent or legal guardian, if under 18 years of age.
Interested Volunteens can sign up for the waiting list until Wednesday, March 17, 2010. You can either call the Volunteer Services Department at 1-262-257-3309 or provide your name, address, phone number, age, name of high school, and year in high school to us using our Contact Us form.
