

Community Outreach Health Clinic serves those who cannot afford private health care including:
New patients are required to bring photo identification. All patients are required to bring proof of income, such as prior year Federal Income Tax Return or most recent payroll check stub to each visit. Financial history is reviewed to determine eligibility to receive services at each visit.

Appointments at the Outreach clinic are available Tuesday and Thursday evenings from 5-8 pm. To make an appointment, call 262-257-3393. Walk-in patients will be accepted between 5:30 — 6:30 pm only.
