Insurance Information

Community Memorial Hospital accepts many insurance plans. We will bill your insurance company on your behalf if you have provided us with your insurance information at the time of registration. If you did not have your insurance information with you, please fax a copy of your card to us at 414-777-0099. You may also mail a copy of your card to the Menomonee Falls address above. Include "Attn: Patient Accounting" in the address. All charges not paid by your insurance company, including coinsurance and deductible amounts must be paid within 30 days of billing unless you contact us to establish payment arrangements.

Financial Counseling/Financial Assistance Program

Financial Counseling
A Financial Counselor is available to discuss payment options with you and, if necessary, set up mutually agreeable payment plans for your hospital bill. If you have no insurance, our Financial Counselors will also review with you programs for which you may qualify and will assist in the application process. You can reach a financial counselor at 262-257-3821.

Financial Assistance Program
Community Memorial Hospital will make available upon request an application for our Financial Assistance Program to any patient or responsible party. If you apply for financial assistance, you will be required to supply, at a minimum, a copy of your most recent federal tax return and a recent check stub (if applicable). This will assist us in determining if you qualify for this program. Federal Poverty Guidelines are used to determine eligibility. Elective services provided by the hospital are not eligible for this program. How do I receive an itemized billing?
As a cost saving measure, Community Memorial Hospital does not routinely send out itemized billings. If you would like a copy of an itemized statement, please contact our Patient Accounting Department at 262-257-3850.

Why am I receiving multiple bills for my hospitalization?
Because Community Memorial Hospital does not employ their physicians, your bill from Community Memorial Hospital does not include any physician billing. For questions on your physician billing, please contact the number listed on the statement. The listing below is a short list of frequently requested numbers:

How can I pay my bill?
You can make payment by cash, check, money order, Visa, Mastercard, or Discover Card. If you would like to provide credit card information over the phone, please call 866-223-6836.

What type of payment plans are available?
Payment plans differ based on patient balance. Please contact us at 866-223-6836 to set up a payment plan on an existing account. For information on future services, our Financial Counselor will be able to assist you at 262-257-3821. Partial payments without an agreed upon payment arrangement will not keep your account in good standing.

The address to send payments to is not in Menomonee Falls. Is this correct?
Yes, the address at PO Box 3297, Milwaukee, WI 53288 is the correct address to mail payments to. This is a lockbox location that Community Memorial Hospital utilizes to deposit payments.

How do I obtain an application for financial assistance/fee reduction?
Please contact our Financial Counselor at 262-257-3821 for information on our financial assistance program.

I did not have my insurance information with me on the day of my services. How will my insurance be billed?

How do I obtain a price quote for a service I want to receive at Community Memorial Hospital?
Our Financial Counselor can be reached at 262-257-3821 and will be able to provide an estimate for future services to you. All quotes are estimates only based on historical information. Actual patient balances will differ due to medical history, lab work needed, pharmacy dosage amounts, etc. Any differences between estimates given and actual balance are the patient's responsibility.

I received a letter from my insurance company or Community Memorial Hospital indicating other insurance information is needed or accident information is needed. What should I do?
You need to contact your insurance company to provide them with the requested information. Many insurance companies require a written response and will not take information verbally. It is your responsibility to follow-up with your insurance company as Community Memorial Hospital cannot act on your behalf in these cases to obtain payment.

I have a previously unpaid balance that is now listed with a collection agency. How can I remove this item from my credit report?
Once an account is listed with a collection agency, you will need to contact them to arrange payment. Once paid, a satisfaction letter will be issued against the credit listing.

How much will Medicare pay for my services?
Due to the complexities of Medicare reimbursement, we are unable to give you a quote on the amount Medicare is going to pay. We recommend calling the Medicare beneficiary support line at 800-633-4227.
