Following heart surgery or a heart procedure, patients undergo a comprehensive program of exercise, risk factor modification and heart health education in the Cardiac Rehabilitation Program. Individual rehabilitation plans are designed to help patients get back on their feet. The program combines state-of-the-art exercise equipment with individualized patient education and exercise guidelines provided by certified staff.
Our professional team includes registered nurses, exercise physiologists, registered dieticians and behavioral health experts. Our Cardiac Rehabilitation program is certified through the American Association of Cardiovascular and Pulmonary Rehabilitation.
Cardiac Rehabilitation is composed of three phases: inpatient, outpatient and maintenance. Your rehabilitation program will begin while you are in the hospital. It is important to follow the nurses' instructions regarding range of motion, movement and walking. Prior to discharge from the hospital you will have a visit from an exercise physiologist or RN from the Cardiac Rehabilitation department to discuss your individualized cardiac rehabilitation plan. This is a good time to ask questions and discuss your concerns regarding Cardiac Rehabilitation.
The outpatient phase of the program will begin one to several weeks following discharge from the hospital. During the outpatient phase, specially trained staff will work with you to help you advance in your exercise capacity and learn more about heart disease, diet, exercise and taking care of yourself.
Exercise sessions meet three times a week at a set class time you choose from the available four class times. Your exercise prescription is individually designed to help you regain cardiovascular health and musculoskeletal strength. Each class session lasts approximately one hour. Program length is determined by both exercise and educational goal attainment.
Education is the key
In addition to exercise, Cardiac Rehabilitation provides heart health education. Our health care professionals provide vital information about heart disease, risk factor management, proper nutrition and weight management, understanding exercise, heart disease treatments and stress management.
Patients seeking cardiac rehabilitation services at Community Memorial Hospital must first get a prescription from a cardiologist or from his or her primary care physician. Classes meet Mondays, Wednesdays and Thursdays. Class times are:
Prior to starting a cardiac rehabilitation program, contact your insurance company to verify coverage. Medicare will cover the diagnosis of heart attack, stent placement, by-pass surgery or stable angina.
